5.12 PHOTODAMAGE/ SUN DAMAGE
Everyday we are exposed to sunlight. Frequent exposure to sunlight since the childhood leads to cumulative effect on the skin & so skin of exposed part become darker. During most of our life 80% exposure to sunlight is before age of 20 years. So most of us has some photo damage on our face & exposed part of the hands, neck & legs. Ultra violet rays in sunlight develop damages in the skin in the form of skin laxity and leathery, deep wrinkles, pebbly coarse skin texture. One can see dark thin to thick layers on forehead, area around mouth, area adjust to lower part of the nose. As this damage increases skin becomes more thin, dry, rough, dyspigmented. Its bruise ability increases. In beauty these areas are to be most bothered. Those who are working in field duty more likely to get affected at early adult age.
Types of photodamage:
Sun damage leads to mottled pigmentation, acne scarring, skin laxity, enlarged pores and altered skin texture. Clinically, photodamage is classified as Type I, II and III. In type I photodamage, telangiectasis, solar lentigenes develop. Indian persons skin often show pigmentation on forehead, around eyes/ mouth, nosalabial fold etc. So, also there is increase in coarseness of the skin and symptoms of rosacea. mild darker areas increase further in thickness and wrinkles. In type II, photodamage skin laxity, rhytides dermatochalasis and comedone develops. In type III, photodamage actinic keratoses, melanoma and non melanoma skin cancer develops. Number of moles, DPN spots (dermato populosa nigra) increase further.
Protect the skin from harsh sunlight: How to protect from sunlight:
People with dry skin should use sunscreen having SPF 15 or more 20 minutes before exposure to sun. Use umbrella or wide brimmed hat. Cover face with thicker cloth. It act as best sunscreen at no cost. On the face sunlight damage is more prominent in perioral (around mouth), periorbital ( around eyes ), glabellar ( area just above nose )and naso labial area.
Treatment: In mild to moderate damage in laser facial (Area adjacent to lower part of nose) rejuvenation is done in one to three treatment sittings with Q-Switch Nd: YAG laser of nano technology. In moderate to severe photo-damage Ultra-Pulse® fractional Co2 laser skin resurfacing is done in one to three treatment sittings Considering size, location, expectation of patients, method of surgery is decidedfollowed by Q switch laser facial rejuvenation. (4 to 10 sittings)
